Rheumatic mitral stenosis (MS) is common in India. Ventricular septal defect (VSD) is the commonest congenital heart disease seen clinically. The incidence of right coronary artery (RCA) arising from the left aortic sinus is 0.17 %. The presence of all the three lesions in a patient is likely to be extremely rare. We present a patient who had all the three lesions, and to the best of our knowledge, this is the first such case to be reported. © 2014, Indian Association of Cardiovascular-Thoracic Surgeons.
