Conservative surgery in treatment for laryngeal carcinoma

The aim of the study was to evaluate the treatment results after conservative surgery performed in patients with laryngeal carcinoma and to analyze the failures. A retrospective analysis was carried on 325 subjects treated by partial laryngectomies in single institution. Disease-free survival (DFS) probability, estimated by Kaplan Meier product-limit method, for the whole group reached 83%, 75% and 63% at 3-. 5- and 10 years, respectively. Five-year DFS was the best for vertical laryngectomy - 86%, then for hemilaryngectomy - 85%, for cordectomy - 80%, for fronto - lateral laryngectomy - 68% and for horizontal laryngectomy - 48%. Seventy four failures (22,8%) were diagnosed. Local recurrences were observed in 13,2%, regional recurrences - in 2.8%. distant metastases- in 1,8%, second primary neoplasm - in 2.59%. and multifocal recurrences - in 2,5% of cases. In conclusion it was stated that the best oncological results in treatment for laryngeal cancer by partial laryngectomies had been achieved in vertical operations and the less effective - in supraglottic horizontal laryngectomy.
