Acute coronary syndrome without persistent ST-segment elevation - what is new in the 2020 ESC guidelines?

摘要：
In August 2020, the European Society of Cardiology (ESC) Guidelines for the management of acute coronary syndrome without persistent ST-segment elevation (NSTE-ACS) were published. It has some relevant changes for clinical practice. Regarding diagnostic workup, the use of fast algorithms based on high-sensitivity cardiac troponin is recommended. Regarding risk stratification, the intermediate risk group has been removed. Low-risk patients should be treated similar to patients with chronic coronary syndrome. A greater individualization of the duration of antiplatelet therapy is recommended. Some changes have been introduced for the invasive management. However, the very high-risk group should still undergo invasive assessment immediately and the high-risk group should receive coronary angiography within 24 hours. New sections deal with spontaneous coronary artery dissection (SCAD) and myocardial infarction with non-obstructive coronary arteries (MINOCA).
