Introduction: Cow's milk protein allergy (CMPA) is the most common food allergy in the first years of life. It is often associated with the introduction of cow's milk formula and is a rare disorder among breastfed babies. Objective: To report the case of CMPA in an infant under exclusive breastfeeding. Case report: A 3-month-old infant presented with a history of chronic diarrhea. The mother denies any introduction of milk formula and the infant is exclusively breastfed. Cow's milk-specific IgE antibodies concentration is in favor of CMPA. In asking the mother again, she points out the consumption of a large quantity of dairy products. Maternal free diet was associated with a normal infant development without intestinal problems. Conclusion: CMPA can develop in exclusively breastfed infants. Excluding cow's milk from the mother's diet is the only cure when she still wants to breastfeed.
