Transpalatal osteotomy technique for midface distraction osteogenesis

摘要
Background: We present our new technique for midface distraction osteogenesis in patients with severe orbito-maxillary hypoplasia. Objective: Using this new type of transpalatal approach, we performed pterygomaxillary disjunction and osteotomy of the medial and posterior wall of maxillary sinus under direct vision. Methods: Twenty-seven patients underwent midfacial advancement using this technique; twenty-two Le Fort III, 3 Le Fort II and 2 Le Fort III / II advancement. Results: Proptosis was lessened and facial proportions were significantly improved in all patients. Serious complications were not encountered. Conclusion: This transpalatal osteotomy technique has an advantage to preserve maxillary first and second molar tooth germ, which is especially important in young patients.
