Acute pain routinely accompanies injury and inflammation, and it may increase the neuroendocrine response to maladaptive proportions. Many therapeutic options now exist that relieve this pain, and perhaps the most serious impediment to proper therapy is failure to recognize its presence. Significant pain should be suspected in any patient with injury or acute illness, and treatment should be provided. This article presents the most effective of the available therapeutic options and how to apply them safely to these critically ill patients.
