摘要：
The elderly population In Brazil follows the world trend and has been provoking the enlargement of the top of the demographic pyramid. Aging is surrounded by physical, functional and psychological changes, and early detection and intervention con minimize the impacts of aging. The objective of this study was to report the Nutrition academic experience in the care of 18 older elderly or equal to 60 years in a Comprehensive Geriatric Assessment instrument(AGA) at the University of Maturity (A) of the Federal University of Tocantins. This is an experience report based on an extension project that included gerontological core. Through the results obtained from the application of the questionnaire, an individualized care plan was elaborated in a detailed way, in the from of a booklet, by a multiprofessional team made up of academics from the courses of Nutrition, Nursing and Medicine. Multidisciplinary teamwork provided broader view of elder care.
