摘要
Endothelial dysfunction is present in the preclinical phase of vascular disease and predicts cardiovascular events in patients at risk for coronary artery disease. Activation of the endothelin system results in coronary vasoconstriction in patients with coronary artery disease. Moreover, endothelin-1 (ET-1) raises blood pressure, induces vascular and myocardial hypertrophy and acts as the natural counterpart of nitric oxide (NO), which exerts vasodilating, antithrombotic and antiproliferative effects. Endothelin-1 is enhanced and has been demonstrated to be a prognostic marker in patients with acute ischemic syndromes and myocardial infarction. Thus, restoring the balance between the endothelin system and the NO system seems to be an attractive therapeutic strategy in coronary artery disease.
