Comorbid substance abuse is a common problem in psychiatric patients, and may contribute to treatment refractoriness, noncompliance with treatment, and increased health services utilization and expenditure. Rates of substance abuse are highest in patients with schizophrenia and bipolar disorder. This review discusses recent findings with regard to the epidemiology, neurobiology, treatment, and health services delivery for comorbid psychiatric and substance abuse disorders.
