Dengue Fever and Aedes aegypti in indigenous Brazilians: seroprevalence, risk factors, knowledge and practices

ObjectiveDengue remains an important public health problem in Brazil. We estimated the associated factors of dengue seroprevalence among native Indians of the Tremembe ethnic and their knowledge about the aspects related to the presence of mosquitoes of the genus Aedes. MethodsAn analytical cross-sectional study and a prospective environmental study to monitor the trapping of mosquito eggs monthly were performed. The serological portion of the study involved indigenous people living in the village of Tapera in northeastern Brazil. Ovitraps were monitored for 12 months. ResultsTwo hundred and ninety of 350 indigenous people (82.9%) participated in the study, with an average age of 30.2 years. The seroprevalence was 22.1% and positivity increased with age, with rates of 4.2% in children under 15 years of age, 26.8% in 15 to 59-year-olds and 42.3% in those older than 59 (CI: 2.25-15.96; P < 0.001). A higher incidence of moving to the city and the presence of underlying diseases were associated with the occurrence of dengue (P < 0.001). Four serotypes were detected, with the highest prevalence of DENV-1 (77.8%), followed by DENV-2 (70.4%), DENV-3 (14.8%) and DENV-4 (11.1%). Eggs were collected in all months of the year and in the traps located in the vicinities of the domiciles (57%). ConclusionsWe present the first seroepidemiological survey of dengue conducted among indigenous populations in Brazil. This lack of studies is likely due to the great bureaucratic challenge of working with indigenous populations, which may lead to greater negligence in the health of these populations. Abstract ObjectifLa dengue reste un probleme de sante publique important au Bresil. Nous avons estime les facteurs associes a la seroprevalence de la dengue chez les natifs indiens de l'ethnie Tremembe et leurs connaissances sur les aspects lies a la presence des moustiques du genre Aedes. MethodesUne etude transversale analytique et une etude environnementale prospective pour surveiller le piegeage des OEufs de moustiques mensuellement ont ete realisees. La partie serologique de l'etude concernait des populations autochtones vivant dans le village de Tapera, dans le nord-est du Bresil. Les pieges a OEufs ont ete surveilles pendant 12 mois. Resultats290/350 autochtones (82,9%) ont participe a l'etude, avec un age moyen de 30,2 ans. La seroprevalence etait de 22,1% et la positivite augmentait avec l'age, avec des taux de 4,2% chez les enfants de moins de 15 ans, 26,8% chez les 15-59 ans et 42,3% chez les plus de 59 ans (IC: 2,25-15,96; p <0,001). Une incidence plus elevee du deplacement vers la ville et la presence de maladies sous-jacentes ont ete associees a l'apparition de la dengue (p <0,001). Quatre serotypes ont ete detectes, avec la plus haute prevalence pour DENV-1 (77,8%), suivie par DENV-2 (70,4%), DENV-3 (14,8%) et DENV-4 (11,1%). Les OEufs ont ete collectes au cours de tous les mois de l'annee et dans les pieges situes dans les environs des domiciles (57%). ConclusionsNous presentons ici la premiere enquete seroepidemiologique sur la dengue menee aupres de populations autochtones au Bresil. Cette lacune dans les etudes est probablement due au grand defi bureaucratique pour travailler avec les populations autochtones, ce qui peut conduire a une plus grande negligence dans la sante de ces populations.
