The management of septal deviations is one of the major challenges for the surgeon due to variability of the etiology of nasal obstruction. In parallel with the advances in rhinoplasty techniques during the last three decades, the treatment options for septal deviation increased tremendously. In this article, a new anatomical and virtual division of the nasal septum known as segmental division is proposed. Additionally, the management of deviations in the caudal and/or dorsal segment is presented. In traumatic and revision cases, more difficult techniques necessitating the harvesting of conchal or costal cartilages are also demonstrated.
