Seeking the optimal aspirin dose in acute coronary syndromes

We reexamined the Antiplatelet Trialists' Collaboration data (11 studies) to evaluate optimal dosing of aspirin for secondary prevention after acute coronary syndromes, while adjusting for patient population and temporal trends. Although aspirin reduced the risks of mortality (odds ratio, 0.82; 95% confidence interval, 0.71 to 0.96), death or myocardial infarction, and death, infarction, or stroke, unadjusted analysis suggested greater benefit in unstable angina versus myocardial infarction and with higher versus lower aspirin doses, highlighting the need for larger, randomized comparisons of aspirin dosing in these patients.
