Marital sex among people living with HIV receiving antiretroviral treatment in Northern Thailand

Before the advent of effective antiretroviral treatment (ART), the sexuality of people living with HIV was mostly discussed in terms of risk. To assess the extent to which ART allows people living with HIV to regain a regular sexual life, we surveyed all HIV-infected people treated in four hospitals in Northern Thailand and a control group from the general population matched by sex, age and residence. Data included socio-demographic and health characteristics, frequency of sexual intercourse in the last month and condom use. Our findings indicate that people living with HIV less often live in steady partnership (50% of the HIV-infected people versus 79% of the controls). After adjusting for factors known to influence sexuality, their probability of being sexually active was estimated to be about half that of the controls. When sexually active, men had a reduced sexual activity compared to controls (2.8 intercourse in the last month versus 4.0), while levels of reported sexual activity were similar among women (2.2 versus 2.8, respectively). Consistent condom use was high among people living with HIV (66% for women and 70% for men). Antes de que existiera el tratamiento antirretroviral (tar), se analizaba la sexualidad de las personas que viven con el vih en terminos del riesgo que ello implicaba. Con el fin de evaluar hasta que punto el tar permite que las personas seropositivas recuperen su vida sexual habitual, los autores del presente articulo aplicaron una encuesta a personas seropositivas en cuatro hospitales del norte de Tailandia, a fin de compararlas con las aplicadas a un grupo de control, seleccionado de la poblacion general, similar en terminos de sexo, edad y lugar de residencia. La informacion recabada dio cuenta de las caracteristicas sociodemograficas y de salud de los entrevistados, asi como de la frecuencia de realizacion del acto sexual durante el mes anterior y del uso del condon. Los hallazgos encontrados indican que las personas seropositivas tienen menos relaciones estables (50 % de las personas seropositivas vs. 79 % del grupo de control). Tras tomar en cuenta los factores conocidos que influyen en la sexualidad, se calculo que la probabilidad de que las personas seropositivas tengan vidas sexuales activas es la mitad de aquella reportada por las personas del grupo de control. Cuando son activos sexualmente, los hombres seropositivos tienen una actividad sexual reducida en comparacion con la del grupo de control (2.8 actos sexuales en el mes anterior vs. 4.0), mientras que las mujeres seropositivas muestran una actividad sexual similar a la de las mujeres del grupo de control (2.2 vs. 2.8, respectivamente). El uso del condon es elevado entre las personas seropositivas (66 % de las mujeres y 70 % de los hombres lo utilizan). Avant l'arrivee des traitements antiretroviraux efficaces (ARV), la sexualite des personnes vivant avec le VIH (PVVIH) etait principalement prise en consideration relativement aux risques sexuels. Afin d'evaluer dans quelle mesure les ARV permettent aux PVVIH de retrouver une vie sexuelle normale, nous avons interroge toutes celles qui etaient suivies dans quatre hopitaux du Nord de la Thailande ainsi que des personnes representant la population generale (groupe controle), appariees par sexe, par age et par lieu de residence. Les donnees recueillies concernaient les caracteristiques sociodemographiques et en rapport avec la sante, la frequence des rapports sexuels au cours du dernier mois et l'usage du preservatif. Nos resultats indiquent que les PVVIH sont les moins susceptibles de vivre dans le cadre de partenariats stables (50 % des PVVIH vs. 79 % des personnes dans le groupe controle). Apres ajustement pour les facteurs d'influence connus de la sexualite, la probabilite que les PVVIH soient sexuellement actives etait estimee a environ la moitie de celle rencontree chez les personnes du groupe controle. Parmi les PVVIH sexuellement actives, les hommes avaient une activite sexuelle reduite, comparativement a ceux du groupe controle (2,8 rapports dans le dernier mois vs. 4), une difference qui n'a pas ete constatee pour les femmes (2,2 vs. 2,8, respectivement). L'usage regulier du preservatif etait d'un taux tres eleve parmi les PVVIH (66 % chez les femmes et 70 % chez les hommes).
