Improvements in operative techniques and resuscitation strategies now allow the severely and multiply injured patient to survive long enough to exhibit extreme derangements in physiology that would have been considered unusual in the past. The triad of hypothermia, coagulopathy, and acidosis now is encountered often in this setting. This article describes the evaluation and management options currently used to manage patients with this deadly triad.
