Hemifacial spasm as the initial manifestation of childhood cerebellar astrocytoma

We report a case of spontaneous orbicularis contractions as part of a subtle hemifacial spasm in a child with cerebellar astrocytoma. A 28-month-old boy presented with a left hemifacial spasm. The patient showed isolated spontaneous contractions about the orbicularis area. Magnetic resonance imaging showed a large posterior fossa tumor compressing both the brainstem and cerebellum. A biopsy revealed anaplastic astrocytoma. Treatment consisted of surgery and radiotherapy. The spasm gradually became less apparent over the of 2 years. Hemifacial spasm in childhood is rare and can be an initial manifestation of a posterior fossa tumor.
