Results of surgical management of primary cholangiocarcinoma of porta hepatis

Twelve patients with Klatskin tumor were treated in the period of 1987-1996 According to TNM scale the stage of rumors was: T1N0M0 - 1 case, T2NoMo - 2 cases, T3N1M0 - 5 cases, T3N1M1 - 4 cases. One patient died before the operation. Among other 11 patients, 5 had excision of the common hepatic duct and the surrounding tissue following hepaticojejunostomy or hepaticoduodenostomy. Explorative laparotomy as a simple procedure was done in 4 patients. They had stents left in the biliary tract before the operation. In one patient a drainage of YT type was applied, and in another transhepatic drainage was done. Two patients had recurrence of cancer and were reoperated on (another hepaticojejunostomy with stents in lobular ducts in both cases, and next, in one case right hemihepatectomy was done). The survival time was 3 - 13 months.
