Association Between Severe Dental Caries and Child Abuse and Neglect

Purpose: In their maxillofacial practice, the authors have encountered some children with severe dental caries whose teeth had to be removed; many later appeared to be abused children. The authors hypothesized that in the group of children who underwent multiple tooth extractions for caries under general anesthesia, a larger percentage would be found to be abused compared with the normal population. Patients and Methods: The authors identified children who underwent multiple tooth extractions under general anesthesia in a well-defined region in the Netherlands in 2005 and 2006. Subsequently, they sought these children in the database of the Dutch national organization against domestic violence and child abuse (Veilig Thuis) in 2015. Results: Of the total group of 376 children, 205 (55%) underwent the procedure because of caries during this period. Child abuse and neglect was established by Veilig Thuis in 47 of these children (23%; 95% confidence interval, 20-26), whereas the procedure occurred before the child abuse was established in 27. Conclusion: There appears to be a strong association between severe dental caries and child abuse and neglect. Hence, severe dental caries could be regarded as an early symptom of child abuse and neglect. (C) 2017 American Association of Oral and Maxillofacial Surgeons
