Psychopathology, psychodynamics and differential diagnosis of early anankasm<< - In the class of disorders susceptible of psychotherapeutic treatment, it is possible to distinguish clinically between classical obsessional neurosis and an early form of anankastic disorder. The author discusses various aspects of compulsive-obsessive illness in early disorders, deriving at least five features from the psychopathology and structural dynamics of ego disorders. It transpires that, at a low level of structure, compulsion/obsession is not primarily pathological but performs a >>quasi physiological<<, ego-sustaining function. There is also evidence of a residual anankastic symptomatology<< in almost all significant structure-dynamic and psychopathological aspects of ego-structural disorders. This assigns early obsession/compulsion a central role in the understanding of those disorders and their manifestations.
