Multiple retrospective studies suggest that there may be a survival benefit to surgical resection of the primary site of a non-small cell lung cancer (NSCLC) and of a single (oligometastatic) M-1 site of disease. There is only one published nonrandomized trial addressing the effectiveness of surgical resection of the primary site and an M-1 site of NSCLC, which did not show a benefit of adding surgery to systemic chemotherapy. Prospective trials are likely to be difficult to complete because of multiple factors; possible trial designs are suggested.
