Adherence in long-term therapies is analysed from the point of view of intentionality, i.e. the role of the formation of an intention in the decision to carry out a therapeutic action such as taking a tablet. We show that non-adherence can be intentional, the consequence of an active decision by the patient (here, the intention being not to take the tablet), and non-intentional, a passive phenomenon that does not depend on his or her will, such as simple forgetfulness. Conversely, adherence is most often intentional, the aim being to avoid complications of the disease. But it can also be unintentional and performed out of habit, leading to the concept of non-intentional adherence. This reflection on the place of intentionality in adherence has practical and ethical implications. (C) 2021 l'Academie nationate de medecine. Published by Elsevier Masson SAS. All rights reserved.
