摘要
with the increase of population aging in our country, the number of patients with chronic diseases is increasing rapidly. Most chronic diseases cannot be cured thoroughly and will burden the patient's family and the society. Chronic disease treatment is a long-term process and the scientific management can not only play an active role on the treatment of patients, but also reduce the incidence of the complication of the chronic disease after the treatment. This paper proposes the health and chronic disease assessment management system. The structure of this system includes four layers, such as data storage layer, data service layer and so on. This chronic disease assessment management system consists of six function parts, which includes patient personal information management, doctor at the community hospital personal information management, doctor at the center hospital personal information management, patient physiological information management, patient condition analysis report management, and role and system security management.
