Clinical case: A 17-year-old presents in transfer from her pediatrician. She reports no health problems, but her mother is worried about her weight, which has been high since menarche at 13. Her menses, about three times per year, are heavy and unpredictable in timing but are not associated with cramping or pain. Her BMI is 29, and she has mild acne and some hair growth around her nipples. Is this normal? Should she be evaluated for polycystic ovary syndrome (PCOS)? Do you need to order any tests? How should she be treated?
