Follow up study of endogenous insulin secretion in GAD-Ab (+) NIDDM. Report of West Japan (Kyushu, Yamaguchi, Osaka) study for GAD-Ab (+) diabetes

A longitudinal study of GAD-Ab (+) NIDDMs from diagnosis to insulin-dependency is underway in Japan since 1996. A total of 2,658 Japanese NIDDMs treated by OHA and / or diet were randomly selected from 15 centers between 1996 and 1998. We detected the GAD-Ab in 54 (2.0%) patients. We have followed the endogenous insulin secretion at 75g OGTT in 10 out of 54 GAD-Ab (+) patients. Decreased insulin secretion during the follow up period was observed in 7 patients and no change in 3. We compared the clinical characteristics between these groups at registry. No significant difference, such as BMI, HbA1c at the registry and duration of diabetes, was observed except for GAD-Ab titer (160 +/- 108 vs. 32 +/- 28 U I ml, p <0.05). There was no significant correlation between GAD-Ab titer and Delta CPR during the follow up period. These results indicated that the endogenous insulin secretion in GAD-Ab (+) NIDDM decreased in short period and the high titer of GAD-Ab detected in NIDDMs would be an early marker for insulin dependent state.
