New studies led to major changes in recommendations concerning remobilisation therapies following stroke. For early re-establishing of stance and gait following stroke a multidisciplinary team approach is recommended. Controlled studies including early mobilisation in stands and training with partial body weight support on treadmills and "gait training" systems showed superior results compared to traditional treatment strategies. Beside adequate management with orthosis drug treatment is another important part of this modular treatment approach. In case of spasticity and e. g. equinovarus and stiff knee pattern following stroke botulinum neurotoxin A injections and/or casting enables to achieve adequate alignment of ankle for stance phase and allow in case of stiff knee pattern to improve joint mobility during swing phase when restricted.
