Safety of Trimetazidine in Acute Stemi and Influence on Exercise and Echocardiographic Parameters

Trimetazidine (TMZ) is a drug which as a cytoprotective anti-ischemic agent improves myocardial glucose utilization through inhibition of fatty acid metabolism. A series of controlled studies (TRIMPOL-II, SELLIER, VASCO) in angina patients have shown that TMZ increases coronary flow reserve, thereby delays the onset of ischemia associated with exercise. To assess the safety and effectiveness of TMZ in the early postinfarction period, we compared standard EchoCG and Tredmille-exercise parameters between two groups of STEMI and non-PCI patients - with standard treatment vs. those who additionally took 70 mg/day Trimetazidine. 2-week follow-up of those groups demonstrated some benefits of Trimetazidine in the acute phase of STEMI on being safe enough, improving exercise tolerance and increasing the thresholds of myocardial ischemia.
