Retrocaval ureter occurs in approximately 1 in 1,000 persons. It is a venous anomaly in which a persistent right subcardinal vein traps the ureter behind the inferior vena cava. Tc-99m MDP bone imaging performed for bone pain in a 54-year-old man showed an S-shaped deformity with dilatation above the midureter on the right side. MR urography suggested a retrocaval ureter (type 1, long loop) on the right side, which was confirmed at surgery.
