Distal Metatarsal Osteotomies for Chronic Plantar Diabetic Foot Ulcers

In more than 30 years of scientific literature (1986-2021),24 the few published studies on the management of CPDFUs by DMOs showed satisfactory short-to medium-term clinical and radiographic results. Although the reports systematically analyzed were all case series (level IV),24 their data suggest that the DMOs, performed at different levels of the distal MBs, with very few contraindications (severe ischemia, gangrene, exten-sive soft tissue infection, and cellulitis) and low incidence of complications (excluding swelling), are an effective surgical treatment option for achieving rapid healing of CPDFUs (within 2 months) and preventing their recurrence after balancing of pres-sures in the forefeet of both type-1 and type-2 DPs.
