Aims - To provide a veterinary behaviourist forms updated according to the current Law. Analysis of the current paradigm of veterinary medical liability must necessarily contemplate the activity performed by a veterinarian behaviorist, the figurehead in prevention, diagnosis and treatment of behavioral disorders. Behavioral therapies are de facto a "veterinary medical act" which may cause civil or criminal liability in case of injury or death of the animals in care. The inquiry about the professional responsibility, as known, always involves the investigation of professional behavior in relation to the particular case. A complete medical record is not only a contractual and ethics obligation, but also shows the professional level of care taken in the evaluation of the animal, its prognosis and therapy, allowing in case of dispute in a court an easy reconstruction of any liability related to the activity provided. This paper aims to suggest reference to professional patterns for the correct preparation of health records.
