Ga-67 citrate uptake in bilateral endogenous endophthalmitis

A 58-year-old woman was hospitalized with sepsis and visual loss. An abdominal CT revealed a hepatic abscess, which was surgically drained, and Klebsiella pneumoniae was cultured from the aspirate. Ga-67 citrate imaging, which was performed to screen for metastatic sites of infection, showed avid uptake by both eyeballs. This is consistent with the clinical diagnosis of bilateral endogenous endophthalmitis obtained by ocular examination. Ga-67 citrate is commonly used to assess ocular sarcoidosis. Its diagnostic utility has been investigated in malignant ocular conditions such as melanoma and orbital lymphoma. Increased Ga-67 uptake by the lacrimal glands occurs physiologically and may be confused with pathologic ocular uptake.
