Sleeve segmentectomy for bronchogenic carcinoma in a patient with severe chronic obstructive lung disease

We performed segmentectomy combined with sleeve bronchoplasty for a bronchogenic carcinoma extending into the segmental bronchus. A 74-year-old male with cough and sputum had an abnormal mass shadow on a mass screening chest x-ray. He had a long history of chronic bronchitis. Preoperative spirometry revealed severe obstructive pulmonary disturbance with FEV(1.0%) of 33.3. Left upper division segmentectomy and sleeve resection of the left B1+2+3 were performed. The left upper lobe bronchus was anastomosed to the lower division segmental bronchus. He is well and without disease 6 years and 8 months after surgery. Sleeve segmentectomy for lung cancer is a curative and lung-preseving operation for compromised patients.
