Adoption of Magnetic Resonance Imaging Combined with Electroencephalogram Monitoring Under Self-Adaptive Filtering Algorithm in Postoperative Recovery Evaluation of Patients with General Anesthesia

摘要
This research aimed to explore the application of functional magnetic resonance imaging (fMRI) based on self-adaptive filtering (SAF) algorithm in evaluating postoperative recovery of elderly patients undergoing surgery with midazolam and dexmedetomidine (DEX). In this study, first, 87 elderly patients who underwent open reduction and internal fixation (ORIF) for femoral shaft fractures (FSF) were randomly divided into a control group (CG) (n = 43) and an observation group (OG) (n = 44). Midazolam and DEX were used for general anesthesia (GA), respectively. Then, the anesthesia monitor was adopted to monitor the differences of heart rate (HR), systolic pressure (SP), and diastolic pressure (DP) in patients before medication (T1), after medication (T2), before surgery (T3), and after surgery (T4). Then, electroencephalography (EEG) signals of anesthetized patients were monitored by EEG monitors, and the differences of Bispectral index (BIS) at different time points in the two groups were compared after the interference in EEG signals was removed by self-adaptive filters. The posterior cingulate cortex (PCC) was used as the seed region, and fMRI was used to monitor the difference of functional connectivity (FC) coefficient between PCC and medial prefrontal cortex during the transition of awake-anesthesia and pain-anesthesia state in the default network of brain region during anesthesia. Finally, the probability of adverse reactions (ARs) was evaluated. The results showed that the self-adaptive filter constructed in this study can remove the interference of fMRI on EEG signals when EEG and fMRI were used simultaneously; the BIS value in the OG was greatly lower than that in the CG at T2 (P <0.05); the values of HR, SR DP in the OG at T2, T3, and T4 were greatly lower than those in the CG (P < 0.05): the FC coefficients of the patients awake and in pain in the OG were greatly lower than that in the CG (P < 0.05); the incidence of postoperative vomiting, chills, tongue drop, and total ARs in the CG was greatly higher than that in the OG (P < 0.05), which indicated that DEX can effectively reduce the probability of ARs after operation and reduce the incidence of intraoperative awareness (IA) in elderly patients undergoing GA, and can be applied to the anesthesia of elderly patients.
