The number of people living with or after cancer treatment is increasing internationally. The large group of long-term survivors, the so-called cancer survivors, is very heterogeneous in various respects and the resulting need for support is correspondingly individual. Structured survivorship programs aim to identify the different needs and requirements of those who are affected. For better information and communication within the interdisciplinary care network of long-term cancer survivors, the development of an individualized survivorship care plan is recommended, a project that can only be successful in an interdisciplinary team approach.
