Applying Evidence-Based Practice Guidelines in Primary Care: A Heart Failure Scenario

Heart failure (HF) is a complex and progressive condition. HF guidelineedirected therapy has been proven to reduce morbidity and mortality. The purpose of this article is to present and debrief a HF case to demonstrate optimized management using the recent HF guidelines, which discusses staging, medication management, and nonpharmacologic interventions. The guideline emphasizes 4 medication classes, highlighting SGLT2 inhibitors regardless of diabetes status. Nonpharmacological strategies are directed at addressing barriers in HF care. Nurse practitioners play a key role in chronic disease prevention, diagnosis, and treatment using evidence-based interventions to improve patient outcomes.(c) 2023 Elsevier Inc. All rights reserved.
