Oblique lumber interbody fusion (OLIF) allows minimally invasive access to the entirety of the lumbosacral spine through a lateral anterior lumbar interbody fusion (ALIF) approach at L5-S1 and anterior-to-psoas (ATP) approach at L2-5. Benefits for this technique allow for the placement of large interbody cages for indirect decompression and realignment. The use of robotics platforms allows for the efficiency of single-position surgery and placement of screws in the same setting while in lateral decubitus. We continue to expand the role of robotics to show how this enabling technology can facilitate single-position surgery in the placement of longer segment interbody fusions. We present here a case of a 59-year-old man who presented with sagittal malalignment and spinopelvic imbalance, who subsequently underwent a single-position L2-S1 OLIF with L2-ilium posterior spinal fusion with robotic assistance (Mazor X Stealth Edition, Medtronic Sofamor Danek). To the best of our knowledge, this is the first video demonstrating the preoperative software and intraoperative surgical technique of a single-position 4-level interbody placement with posterior fixation to the ilium in lateral decubitus position. There is no identifying information in this video. A patient consent was obtained for the surgical procedure and for publishing of the material included in the video. The participants and any identifiable individuals consented to publication of his/her image.
