center dot In resectable PDAC, neoadjuvant radiation is institution dependent with adjuvant radiation used for patients with close/positive margins, lymphovascular invasion, or multiple lymph nodes. center dot The role of neoadjuvant chemoradiation in borderline resectable PDAC is contested but the ongoing PREOPANC-3 and PANDAS-PRODIGE 44 trials are expected to provide clarification. center dot Advanced delivery techniques such as SAbR or SMART allow for escalation of the biologically effective dose with favorable toxicity profiles.
