The multi-professional health center as part of the primary care landscape

Context. The high drugs consumption pushes the occurrence of adverse effects forward. The general practitioner can prevent this with deprescription. But doctors think patients are seeking a drug prescription. Goal. Explore patients' experiences of deprescribing and motivations for taking medication. Methods. It consisted in a qualitative study with analysis based on an anchored theory approach. Individual comprehensive interviews of patients living in the Pas -de -Calais department have been conducted. The triangulated analysis has been made with the QSR NVivo (R) 11.4 software. The data sufficiency has been obtained after the analysis of 10 interviews. Results. Motivations for taking a medication depended on several factors. Patients perceived a benefit (life comfort vs adverse effects). They had knowledge, beliefs and external influences: caregivers, relatives, advertisement, medication leaflet. There were also specific unconscious motives. The physician had to understand the importance of treatment for the patient and seek eventual side effects or their warning signs. Patients are seeking information because of varying knowledge level or mistaken beliefs. It was necessary to suggest modifying the way of life and treatment - a progressive reduction of posology or alternative medication - reassuring the patient about the possibility to prescribe the initial treatment again if needed. This process could be helped by the context, including the use of a secured prescription or an early deprescription. Conclusion. Studying motivations for taking medication could help the deprescribing process: psychological therapies to reverse a conditioned behaviour, collaboration of nurse, doctor and pharmacist joining their influences.
