血清miR-221-3p及miR-133a-3p在结肠癌中的表达及其临床意义

摘要
[目的]探讨结肠癌患者血清miR-221-3p及miR-133a-3p的表达及其临床意义。[方法]选取收治的160例结肠癌患者(结肠癌组)、108例结肠良性疾病(良性病变组)和60例健康体检正常者(对照组),检测各组血清miR-221-3p及miR-133a-3p表达水平,分析其表达与临床病理特征的关系。应用ROC曲线分析血清miR-221-3p及miR-133a-3p表达水平对结肠癌的诊断价值。Pearson相关分析血清miR-221-3p及miR-133a-3p表达水平的相关性。[结果]结肠癌组血清mi R-221-3p(3.94±1.08)表达水平明显高于良性病变组和对照组(1.51±0.43,1.42±0.36)(P均<0.001),结肠癌组血清miR-133a-3p表达水平(1.85±0.62)明显低于良性病变组和对照组(3.18±0.86,3.27±0.94)(P均<0.001)。结肠癌患者血清miR-221-3p及miR-133a-3p表达水平与TNM分期、分化程度及淋巴结转移有关(P<0.05)。ROC曲线分析显示,血清mi R-221-3p及miR-133a-3p表达水平诊断结肠癌的最佳截断值分别为2.90、2.26,两项联合诊断结肠癌的曲线下面积最大(0.905,95%CI:0.844~0.962),其敏感性和特异度为90.2%和84.5%。相关分析显示结肠癌患者血清miR-24-3p与miR-221表达水平呈负相关(r=-0.742,P<0.01)。[结论]结肠癌患者血清miR-221-3p表达水平明显升高,而血清miR-133a-3p表达水平明显降低,两项联合检测对结肠癌诊断具有一定的价值。
