6th/7th AJCC/UICC胃癌分期系统的比较

摘要：
目的评估6th/7th美国癌症联合委员会/国际抗癌联盟（AJCC/UICC）胃癌分期系统的临床适用性。方法对纳入研究的401例胃癌术后患者分别按照6th/7th分期标准分期。采用线性趋势x2检验、似然比x2检验、赤池信息准则和ROC曲线下面积对两版分期系统进行评估。结果 6thT、N各分期组间5年生存率差异有统计学意义（P<0.05）,7thT分期T1a/b组间差异无统计学意义（P>0.05）。6th/7thT、N分期（5个亚组）AUC比较差异有统计学意义（P<0.05）。6thTNM分期系统赤池信息准则（AIC）小于7thTNM分期系统,AUC比较差异无统计学意义（P>0.05）。结论 7thT分期、N分期优于6th分期标准,6th/7th综合分期系统预后判断价值差异无统计学意义（P>0.05）,6th综合分期系统更简洁直观。
