超声引导下定位胚胎移植对IVF-ET妊娠结局的影响

摘要：

目的:探讨在腹部超声引导下定位胚胎移植对体外受精-胚胎移植(In vitro fertilization and embryo transfer,IVF-ET)妊娠结局的影响。方法:回顾性研究本生殖中心接受首次IVF-ET,包括卵母细胞浆内单精子注射(Intracytoplasmicsperminjection,ICSI)治疗的新鲜胚胎移植232个周期。232个周期按超声下气液滴距宫底距离占宫深比例的不同分为4组,A组(≤1/5)46例、B组(1/5<B组≤2/5)68例,C组(2/5<B组≤3/5)70例、D组(>3/5)48例,分析不同的胚胎移植位置对IVF结局的影响。结果:各组间患者的一般情况如平均年龄、不育年限、平均获卵数、优质胚胎数、移植胚胎数及子宫内膜厚度等均无统计学差异,B、C组的胚胎种植率(25.00%、22.86%)高于A、D组(16.67%、14.58%),差异无统计学意义;B、C组的临床妊娠率(44.12%、42.86%)明显高于A、D组(21.74%、22.92%),差异有统计学意义(P<0.05);各组间异位妊娠率无统计学差异。结论:超声引导下置胚胎距宫底的位置占宫深的比例在1/5~3/5之间,可获得较高的临床妊娠率。
