KN93对心力衰竭兔心室肌细胞迟后除极和钙离子的影响

摘要
目的观察CaMKⅡ抑制剂KN93对心力衰竭兔心室肌细胞迟后除极和钙离子的作用,探讨CaMKⅡ信号通路对心衰后触发性心律失常的影响。方法选取雄性新西兰大白兔30只,随机(随机数字法)分为3组:假手术组(sham组)、心衰组(HF组)、心衰+KN93 (HF+KN93组),每组10只。采用腹主动脉缩窄联合主动脉瓣反流术建立兔心衰模型,利用双酶法分离单个兔心室肌细胞,全细胞膜片钳记录动作电位和瞬时内向电流(Iti)电流,利用离子浓度测定系统检测胞内钙瞬变,用Western blot技术检测主要钙转运蛋白。数据采用pCLAMP10.2处理,SPSS17.0软件进行统计学分析,多组间数据比较用ANOVA方差分析,组间两两比较用SNK-q检验。结果 (1)心衰后兔心室肌细胞发生迟后除极(delayed afterdepolarization,DAD)和触发活动(trigger activity,TA)增加,应用KN93 1.0μmol/L后,DAD和由此引起的TA均降低。(2)Iti电流密度在心衰后增加,在-50 mV时,峰电流密度从假手术组细胞的-0.12±0.02 pA/pF增加至-0.95±0.06pA/pF(n=10,P<0.01)。暴露于KN93 1.0μmol/L后,电流密度降低为-0.44±0.04 pA/pF(n=10,P<0.01)。(3) KN93可以减少胞内钙离子浓度、降低钙瞬变幅值,加速钙瞬变的衰减。(4) KN93可以上调pPLN和SERCA2a的表达,增加胞内钙离子的摄取,而降低NCX的表达,减少Iti电流,抑制DAD和TA发生。结论 KN93可降低心衰动物模型的心室肌细胞内钙离子浓度,有效减少DAD和TA的发生,CaMKⅡ可能成为抗心衰时心律失常发生的新靶点。
