Avoiding form failures

Some cases of avoidable concrete formwork failure are recalled and it is pointed out that contractor's vigilance is required at the job planning and form design stages as well as during the concrete forming and placing. The form designer should plan systems that can be erected by the least skilled worker on the job. At the same time,, safe and correct work procedures should be enforced by jobsite supervision and training. Problem areas are identified and the conflict between economy and safety is considered.
