[4] In a typical ICU, vital signs are taken frequently but may only be charted every 30 or 60 minutes. We are being asked to document the exact time at which the patient first reaches target temperature. Have you come up with a tool or tracking system, preferably something that can be done in EPIC or other electronic record, that nurses can use to assure accurate documentation of the time the patient reaches target temperature?
