Neglected non-united lateral humeral condyle fracture in a ten-year-old child

Lateral humeral condyle fracture (LHCF) is one of the most common fractures of the elbow in children. We describe the case of a 10-year-old boy who presented with a neglected, nonunion LHCF 60 months after the initial trauma. He was treated by open reduction and internal fixation after reducing the lateral condyle to its anatomical location and fixing the fracture using two screws; no addition of a bone graft and no anterior transposition of the ulnar nerve was needed. At the 14-month follow-up, there was complete union of the fracture. The patient achieved full elbow flexion and extension as well as full forearm supination and pronation, and the carrying angle was corrected from a preoperative 140 degrees to 160 degrees at the last follow-up. Proper diagnosis and management of LHCF at the first presentation are crucial; however, if the patient presents with a neglected and nonunion fracture, open surgical fixation leads to satisfactory radiological and functional outcomes. Eine Fraktur des Epicondylus lateralis humeri (LHCF) ist eine der h & auml;ufigsten Frakturen des Ellenbogens beim Kind. In der vorliegenden Arbeit wird der Fall eines 10-j & auml;hrigen Jungen vorgestellt, der 60 Monate nach dem Initialtrauma eine unerkannte Non-Union-Fraktur des Epicondylus lateralis humeri aufwies. Die Behandlung erfolgte mit offener Reposition und interner Fixation durch Zur & uuml;ckverlagerung des Epicondylus lateralis in seine anatomische Position sowie Fixierung der Fraktur mit 2 Schrauben; eine zus & auml;tzliche Versorgung mit einem Knochentransplantat oder einer anterioren Transposition des N. ulnaris war nicht erforderlich. Bei der Nachuntersuchung nach 14 Monaten war es bei der Fraktur zur kompletten Verheilung gekommen; der Patient erreichte eine vollst & auml;ndige Ellenbogenflexion und -extension sowie eine vollst & auml;ndige Unterarmsupination und -pronation. Der Tragewinkel war bei der letzten Nachuntersuchung von pr & auml;operativ 140 degrees auf 160 degrees korrigiert. Die richtige Diagnosestellung und Behandlung der LHCF bei der Erstvorstellung sind entscheidend; wenn sich der Patient jedoch mit einer unerkannten und nicht verheilten Fraktur vorstellt, f & uuml;hrt die offene chirurgische Fixation zu angemessenen radiologischen und funktionellen Resultaten.
