摘要：
Schwere depressive St & ouml;rungen sind weltweit eine der Hauptursachen f & uuml;r Behinderungen, in den USA betreffen sie sch & auml;tzungsweise 21 Millionen Erwachsene. Obwohl Antidepressiva weit verbreitet sind, sind sie bei mehr als einem Drittel der Patienten nicht optimal wirksam. Eine behandlungsresistente schwere Depression wird allgemein als Depression definiert, bei der Patienten auf 2 oder mehr Versuche einer ad & auml;quaten Antidepressivatherapie nicht zufriedenstellend ansprechen. Die Elektrokrampftherapie (EKT) wird bereits seit etwa 80 Jahren zur Behandlung einer therapierefrakt & auml;ren Depression eingesetzt, Ketamin seit etwa 20 Jahren. Die Wirksamkeit im Vergleich beider Modalit & auml;ten ist derzeit ungewiss.
