The wandering spleen is a congenital or acquired anomaly which can be found in all quadrants of the abdomen along the length of its pedicle. It can sometimes manifest as intermittent abdominal pain caused by the torsion of its pedicle. We report a 26 year-old patient who presented with a painful abdominal mass intermittently for over a year. Clinical examination suspected an ovarian tumor. The computed tomography showed a large right abdomino-pelvic spleen without necrosis and the emptiness of the splenic lodge. Its long pedicle showed kinks. The patient underwent a splenectomy due to severe chronic pain and hypersplenism. The evolution was uneventful with disappearance of symptoms.
