A 72-year-old man had a spontaneous perforation of a duodenal diverticula, with intra-abdominal abscess. Perforation is a rare complication of congenital duodenal diverticula. Another serious but rare complication of duodenal diverticula is stone formation. The preoperative diagnosis is often acute cholecystitis, appendicitis, or perforated duodenal ulcer. Operative treatment consists of excision of the diverticulum, closure of the duodenal defect, and drainage of the associated abscess if present.
